Individual Referrals and Supportive Partnerships

Uplifting Dementia has a referral system where “deep dive” consultations focus on providing tailored support for individuals with dementia and/or their care partners.

This involves detailed assessment of the person’s background, needs, preferences, and experiences with dementia, leading to the development of bespoke solutions, and personalised care service and support, to empower both the person living with dementia and care partners.
