Individual Referrals and Supportive Partnerships
The process may include:
1. Assessment and Understanding
- Individualised Needs
Consultation starts with a thorough assessment of a person’s unique circumstances, including their cognitive,
emotional, and physical needs. - Personal History and Preferences
Understanding background, interests, routines, and preferences is crucial when creating a supportive and
meaningful care environment. - Care Partner Support
Addresses the needs and concerns of care partners, who play a vital role in supporting the person with dementia.
2. Developing a Personalised Care Service Plan
- Tailored Interventions
Based on the assessment, a personalised care plan is developed, outlining specific strategies and interventions to address each persons unique requirements. - Non-Pharmacological Approaches
The consultation emphasises non-pharmacological strategies, such as environmental modifications,
communication techniques, and activities that promote well-being and engagement. - Care Partner Education and Support
Care partners receive education and resources to help them understand dementia and implement responsive
support.
3. Ongoing Support and Monitoring
